Here at BTVSAL, we have no one application that we like to use. Frankly, after your first character is approved, all you have to do is tell us about your character and tell us the PB and we're happy. However, I know that some people like to fill out applications. Frankly, this mod likes to do them for almost every character because it gives the ability to figure out a character before playing them. Trust me, it helps.
That being said, there are a few applications that I'm willing to take.
First we have the basic one. Now, the more you write, the less questions we have to ask. We don't do word minimums, though. Also, in any application, if you are applying for a demon or something that isn't common, please either explain fully about the species, or link us to something that we can read.

Next up is one that's a little more in depth, obviously, and it's one that I like. I got it from a friend's game over at Livejournal that I used to play in, Attestatio Vita. Sadly, the game is no longer active, but the application is still more than servicible.
