2. It's a Firefox problem. Here's my support stock answer:
This behaviour happens because of Firefox's autofill feature. There are hidden form boxes for the username and password on the update page, which are filled automatically by Firefox. As this triggers the alternative login mode of the page, the drop-down menus for icons and for asylums to post in don't work.

Although it is beyond the scope of InsaneJournal support to provide support for Firefox, several users have reported the following solutions to work:

By going to Tools > Options > Security > Show passwords, you need to remove the saved password information for InsaneJournal. The next time you log in, when prompted to save the password, you should choose "Not for this site". You will have to type both username and password every time you log in, though.

Alternately, since the autofill feature is only triggered when there is only one username/password to fill, you might want to create dummy information by trying to log in InsaneJournal with a dummy username and/or password and prompting Firefox to remember this password.

3. In beta, so disabled. Hopefully should be working with the next code update. *crosses fingers*

4. Yep, that's in some site schemes, because they date back from the time people didn't have their own subdomain. Not sure whether/when it'll be fixed, though.
