But to do that, I need your help. So if you want to pimp your cast, please fill out this form as accurately as you can.

Please be careful with the formatting. Leave the textarea tag in. (Those of you who have been in the game for a while might remember that that's how the taken page used to work, and it's the same deal here; it's just there to make my life easier.)

Feel free to delete the notes section if you don't have anything particularly important to add; in fact, it'd be nice if you do, since it'd be good to save space. But I thought it'd be good to include it just in case.

Um...at this point, I think the formatting is basically finalized, but I might tweak it later. If you have any thoughts about it, feel free to let me know and I'll try to work with you. I'm not that great at coding, though.
