That's troubling, given the cause and effect rule an entrance to a place should have an exit the same way.

What exactly happens to the worlds we've left?

Oh, it does. Just not one we can use at will. It seems that the entity is in charge of our entrance and exit.

Many people have arrived from different parts of their timelines. And no one has reported that anyone who came from earlier points had disappeared from their world.

Well that's not terrifying.

So it's like leaving the reality at a certain point in time but returning to it at the exact moment when you go back?

That's my experience anyway. I went back ever so briefly, I barely even remember it. And then arrived back here weeks later.
