[taking a deep breath, waiting as long as he possibly can before speaking--]

I wish that the man known to me as Uchiha Madara fails to extract the Kyuubi from Uzumaki Kushina and subsequently dies at the hands of Namikaze Minato when confronted for said extraction attempt, thus preventing the second Kyuubi attack upon Konohagakure and preventing the causes and occurrence of the Uchiha massacre and allowing my younger brother Uchiha Sasuke the best possible chance of living a long and fruitful life...

...and I wish that the effects of the wishes of my teammates and partner-teammates, whether already made or to be made, are negated with regards to myself if the fulfillment of those wishes with regards to myself and myself alone would interfere in any way with the successful fulfillment of my own wish.
