Please find attached Form DoM-135U, Request for Medical Leave (Project-Related), with supporting documentation. I am taking the unusual step of filing this request directly with you because of the concerns I have about my current project and its effect both on me and on other participants, and because of the political nature of the project and its possible outside ramifications.

In accordance with Department regulations, I have not sought outside treatment for the effects documented herein. Unspeakable Scrimgeour has been treating me as part of his own project, as detailed in supporting documentation. He has supported my request verbally and I believe will support it in writing.
