Of course it comes with a fee; why did you think I told you it cost an extra $60? Technically it's supposed to be $759, because it's supposed to be an additional 10%, or $69, not just $60 -- but I'm lazy enough about it to think that $750 looks nicer, instead of worrying about the $9.
I'm the only one with the time or inclination to tailor a piece from scratch, though. Anyone else, you'd get the 10% markup and it would be tailoring from a base piece, instead of from materials.
Most of the time, it turns into the inner liner being made of a different pattern fabric than the off-the-rack models.
