Actually, that link up above doesn't apply to you. They specifically don't train guide dogs for the blind.
There's no actual governing body that regulates service animals in the United States outside of some honestly pretty sparse legalese in the Americans with Disabilities Act. If you dig around, you'll find that different organizations may have different rules and qualifications they use to determine appropriate placement of the dogs.
But the waiting lists can get pretty long, so you may not be matched with a dog for a long time even after you apply, if they approve the application.
