I understand why you find it impossible to get past (there are hundreds of things that can throw me out of a story, some as small as a misspelled word), but I hope my fiction (or anyone else's fiction) is understood not to be a bdsm instruction manual. In real life, I would refuse to accept a submissive who didn't want a safe word. I would assume he or she needed a level of sadism I cannot provide. I would also never disregard a safeword. My personal experiences with bdsm, however, aren't relevant to the story, which is a "what if" scenario: what if John's fear for Rodney's life grew great enough that he deliberately pushed Rodney to an edge, and how would Rodney react? A comparable scenario would be the parent who snoops through a teenage child's personal belongings because of suspected drug use.
