I usually write in OpenOffice (quite the same as Word), then copy the plain text to NotePad (that destroys bold and italic and formatting of paragraphs, but leaves the smart quotes) and then or write tags in NotePad, or in the lj edit window itself. So I can have smart quotes in the text, but not in the code.
I´m usually copy-pasting the text only, not the code. For some more difficult entries I have written blank "mirror" - code only, like < img src="insert address here" / >. (If this can help you.)
