I'm pretty sure that Urich is NOT smoking in the hospital - he's just dangling an unlit cigarette and toying with his lighter. Otherwise, the bit about how he hates hospitals with no-smoking rules would be somewhat pointless. (Also note that there is no trail of smoke coming off the end of his cigarette.)
The Punisher ought to rethink that particular version of his costume. Sure, it looks cool and all, but those 'teeth' on his belt are mighty impractical - as drawn the way they are here, they'd jab into his upper belly every time he tried to bend over. (What are they FOR, anyway? I mean, obviously their main function is to look nifty, but do they actually serve a purpose beyond that, or not?)
And damn, but Tombstone would be seriously creepy in real life.
