Application sheet!

Below is the Application form we used for my old game, Apotheosized. The Layout itself was pretty well liked, and was even used after the game abruptly ended by people who stole it. However, I'd like to open it to the public, for all of you guys!

Feel free to play with it, by changing the colors around, etc. O1 Is an example of my application, for my old game, utilizing the code buy playing around with the color combinations.

NAME Their mythological name.
PANTHEON Roman, Norse, Greek, Egyptian, etc.
ALIAS/NICKNAMES Nicknames, which they are referred by REAL name. A nickname for Aphrodite could be Venus. Odin is often known as Wodin, Anansi is sometimes called Aunt Nancy.
HUMAN ALIAS Name in which they are legally called in the United States.
NICKNAMES Nicknames, which they are referred by HUMAN name.
AGE/DATE OF BIRTH What age do they appear? what is their mortal selves date of birth?
LEGAL STATUS Do they have dual citizenship with a country? Have they had any issues as their human selves?
HOMETOWN Where does it say on their birth certificate they are born?
CURRENTLY RESIDES Current place of residence. If outside of Olympia, state so. If inside, please include where they are living. Dorm room? Apartment? etc.
OCCUPATION If they are employed, where? Are they a student? what year?
SEXUAL ORIENTATION Heterosexual, Bisexual or Homosexual. Other forms accepted. (NOTE* Please stick to canon. Zeus has slept with males, so he could be considered bi, etc.)

FULL EXTENT What could they do when their abilities were in full gear? In the prime of their ages. Please provide in detail what they were capable of. If they gain power, etc -- we'll be playing off this.
ITEMS OF POWER Some gods traditionally held items that bestowed powers on them. Poseidon had his trident, Hermes his shoes.
SKILLS/TALENTS Less godly. Do they sing, can they dance the quickstep? What kinds of talents and skills do they have?

	Commenting so I can use copy and paste on my computer (my cousins laptop is a macbook, and for some reason, doesn't have a right click function) when I sign on later. And also to give my thanks~ This is a very useful template, though I will be changing things around a bit. (Reply to this)
