Dear Mr. Morgan,
In the past weeks, as is often the case with recovery, Marta has seen a number of challenges, including mental, physical, and emotional. While this is extremely common, and we are equipped to handle all facets of recovery, there is a physical matter that is hindering her progress.
She continues to find difficulty with verbal communication, and combined with tremors in her hands that impede her ability to write, it has made interaction with others an increasing hurdle to her progress. As our programs rely heavily on interpersonal work, you can understand that the inability to consistently and accurately communicate is doing her recovery no favors.
My current suspicion is that her overuse of Clonazepam (and possible other substances) may have been causing seizures, leading to possible neural damage. A proper diagnosis will help us in determining the next steps of her care. Therefore, I would like to request that she receive an MRI sooner rather than later.
Due to the situation, we require your consent and approval to transport Marta to our nearby sister hospital in order to perform the needed tests. You will find a number of pdf forms attached to this email. Please complete, sign, and return to us as soon as possible. You can email, fax, or place them in the mail.
Once her test results are analyzed and I can confer with my colleagues, I will contact you again with an updated treatment plan for Marta.
