Who: Moira & Pepper
What: Pepper gets the genius idea to put the reactor into Tony's chest. <-- that's sarcasm.
When: Right after this.

Moira stepped out of surgery, pulling her mask down and exhaling heavily. She waved off a nurse, pulled a new coat on to cover the bloodstains and headed out to the waiting room, looking around for the woman who'd come in with Tony Stark, "Och....Miss Potts?"
She put a hand gently on the woman's shoulder and squeezed.
"Everything's going to be okay," Pepper mumbled just before jerking awake. She had said those words to Tony in her dream as he was going into cardiac arrest. She believed them.
It took Pepper a minute to figured out where she was and what was really going on. "Um. Yes? Yes. I'm Miss Potts," she said groggily. "How's Tony? Is he going to be okay?" she asked.
"I'm Doctor MacTaggert. I'm t'chief surgeon. I was workin' on Mr. Stark."
This was hard. It was always hard. It didn't matter how good a doctor you were, sometimes there was too much damage, no matter what you did.
She kept her voice even, and gentle, "I couldnae remove all t'pellets. There's shrapnel movin' towards his heart an' nothin' we can do tae stop it. Without a miracle he willnae last the night. I'm sorry."
This was all a little too surreal for Pepper. She couldn't believe what she was hearing and the kicker was that she had the arc reactor with her. He always kept it on him. He had it with him at dinner. This woman would probably think she was insane if she suggested such a thing, but it didn't sound like Tony was going to make it out alive.
"What if I told you I knew a way to save his life?" Pepper said despite her better judgement. It seemed she'd been doing a lot of that since meeting Tony.
"I'm listenin'." Moira nodded her head, then gestured for the woman to follow her so that they could speak in private, "I know Mr. Stark has his fingers in a lot o'things. If there's anythin', I'm willin' tae try." She frowned, "I dunnae like it when patients insist on dyin'."
That.... wasn't exactly the reaction that Pepper had expected, but she was going to take it. It sounded like this doctor might be more on the liberal side. If Tony died because of the procedure Pepper was suggesting she could lose her license, but Pepper was beyond caring about that at the moment.
Once they were in private Pepper reached into her purse and pulled out the arc reactor and handed it to the doctor without a word.
Tony was dead anyway. If there was a chance it could save his life, Moira was willing to try it. She had a reputation for being open minded and thinking out of the box. She held the reactor in her hand and looked at it. It was a little too far out of the box for her experience.
"What..what is this?" She lifted it up and peered at it. She could feel its hum. An instrument around her neck vibrated as the reactor pulsed, and she looked down at it, "Och..Are ye suggestin' what I think yuir suggestin?"
Pepper gave a tentative smile to Moira and nodded her head. "I think I am," she said. "This is an electro magnet.. or something. I don't know. The point is that I know it can save his life," she told the other woman.
Moira put the reactor on the table, next to some paperclips, which started to edge away from it, "Och! Yuir right! That's it! This thing can keep t'shrapnel away from his heart!"
She pointed at Pepper, "There's an observation room. I cannae let ye in tae the room, but ye can watch...I think he'd want ye nearby."
