The question in general makes no sense. Or rather, it's ineffective. People lie all the time, mostly because it's a casual, over-used question that both parties normally know is merely a formality and that neither party really cares nor wants to know the real answer.

Therefore, you may ask how I'm doing, and for all you know I could be dying in a hospital wing, but nevertheless, I would still say 'Fine' and then you would say 'Good' and go upon your merry way, none the wiser.
