In my (East Coast USA) experience, I transitioned from Miss to Ma'am at about 30. (Though for some reason I still get Miss if I'm wearing my hair in pigtails.) I found the first few Ma'am's jarring, but I was never offended about it.

I never used miss for anyone adult, or even close. It was always ma'am if I didn't know their name.
