Mr. Queen,
While I understand that you have issues with what is being asked of you, when a person is attempting to be flexible and work with you as much as possible within the confines of the rules both have been given, rudeness is neither helpful nor appreciated.
The exams will require you to remove your clothes to be completed properly, unless you would like me to go about poking my fingers into your rectum without being able to see what I am doing. I assure you, I have been a doctor and medical examiner for many years and the sight of a naked man will not cause me to lose all sense of propriety. Unless you can provide me with a valid medical reason why this physical should not be performed properly, I shall be forced to make the assumption you are hiding a debilitating condition that will adversely affect your ability to perform your duties and report as such to those in charge who will be expecting a reason as to why I cannot clear you for active duty.
I am not your enemy, Mr. Queen, however, there are rules and whether we like them or not, they need to be followed. I am more than happy to find whatever ways I can to make this as painless as possible for you within those rules. But, I cannot help you if you insist upon being both unresponsive and insulting.
