I will be administering routine physical examinations over the next few weeks. Everyone will be examined.

To minimize the conflicts with training, club nights, and assorted family dinners, I will conduct the examinations between the hours of ten o'clock and one o'clock over the next two weekends on both Saturday and Sunday. These will be house calls. I will bring what equipment is needed. Results will be available within one week of your examinations.

I was wondering if you would be interested in playing lab assistant for me in the afternoon/evenings around dinner with your parents for the two weeks I'm running tests. I promise you won't have to do anything with the blood, or anything else but keep me company, really. I need to use the Medical Center here to work and wouldn't want to cause problems going by myself. We can talk about politics and science and all those civilized things no one else wants to talk about here.

Mr. Queen,

Unfortunately, none of those records came through the rift with you to verify that claim. And since that examination, you have been transported through to another dimension, the genetic make-up of your body altered, and are now being prepared for a mission and a future in which there will be no further opportunity for the level of medical treatment available to us here.

While it is your choice, of course, whether to accept medical treatment or not, I have no other way of assessing your health and physical readiness for what is being asked of you. It would be in your best interests and in the interests of those who may depend upon you later, to have a current and accurate assessment of your physical condition before undertaking this rescue mission. I strongly recommend allowing the examination to be completed.

Please at least consider it for a few days before making your decision.

Doug,

It's very simple. I have every confidence you'll be fine. It's much more common to have a response to venipuncture as a patient than the person performing the procedure. But, if you're worried, we can do it before leaving the house for the lab. A bed or sofa should provide a safe enough place should you lose consciousness.

Lieutenant,

While I can understand your reluctance and will, of course, respect your wishes, I would also ask that you please at least consider the choice for a few days in the interim.

I have been told very little about the previous health care available. However, I can assure you that I will take no action that could potentially bring harm to you or your child. My only intention with these examinations is to assess the current well being of the team before it's members are essentially deployed into hostile territory with little or no access to real medical care. There is no treatment involved and would be no treatment in the future that I was not fully qualified to administer.

I am sure the doctor who was here was perfectly adequate at what she was trained to do, there is a great difference in the competency of a surgical resident who is still in the middle of her training and a fully certified pathologist with years of practical experience in varying conditions. As I said, it is certainly your decision, but I would ask that you at least take the days between now and the day I will be coming to your house to consider giving me a chance to prove that proper medical attention can, in fact, be beneficial.

I'm not a Lieutenant. If you are ignorant about ranks, use my name and don't insult me with a rank that I haven't had for decades, Doctor.

You're right, you know very little, so let me enlighten you. The previous doctor was only interested in 'helping' so she could claim to have done whatever she can, in the process almost killing my son, because she had no idea what to do with a preemie.

Every single vaccine or medical test he has needed since birth was conducted by me. She had no problem leaving four other babies without any care when she decided that she didn't like life with the group.

You might be a certified pathologist, but you're not a pediatrician and know nothing about preemies. In fact, I bet more about them than you do. Your tests would likely have false reading if you took into account his gestational age as opposed to his real age. So I don't need another doctor with a God's complex to tell me what she can do to 'help' me, when she knows less than me and that 'proper medical attention' could end up endangering my child.

Is that explanation enough for my decision?

My sincerest apologies. The insult was inadvertent, a result of verifying information prior to the realization the brownies at the club had been tainted. I assure you no such insult was intended.

I do understand your frustration with the previous medical care. Thank you for explaining further. While I cannot explain or excuse the previous doctor's actions, I can further explain my own position to correct certain assumptions in your reply. It may not change your opinion or decision. But, perhaps we may reach a better understanding of each others' positions. I would like to believe my desire to help is genuine and not the result of a God complex as you have called it. However, only the evidence provided by further association can possibly prove that to you.

I am not a pediatrician, but both my specialty in pathology and my post as Chief Medical Examiner do require more knowledge of the human body at all stages of development, including infancy, than most other non-pediatric specialties. As a medical examiner, this knowledge is essential in determining cause of death at all ages. A pediatrician will request tests, this is true, but the person looking through the microscope is still a pathologist and it is upon their summation of the test results a pediatrician will make their diagnosis.

Additionally, a proper physical examination, particularly by a new physician, includes a review of the patients medical history, during which I would have expected you to provide such information regarding the tests and vaccines administered previously, as well as any other conditions or past problems. It is not my intention to come blundering blindly into anything as that would, indeed, be a danger to your child and anyone else I am to see.

However, I will still be at the house to examine others living there and you are welcome to observe my methods if it acceptable to those being examined, excepting of course, the discussions of my patients' medical histories and examinations of a more personal nature. Should you change your mind at any time, my proverbial door is always open.

I am perfectly aware that I have a bias against all doctors at this point, however your rather summary email rather gave the impression that you are stepping in to feel good about yourself since we never had to bother with physicals before.

You might want to ask questions before you send this type of emails, because this is not a group that volunteers information. Being in the sharing spirit, you should probably know that during the month of December, everyone was vaccinated for the following:

- Adenovirus: 2 tablets good for life (Anyone in the military in the past 5 years wouldn't need it)
- Anthrax: 5 doses of vaccine (in the muscle): subsequent doses to be administered at 4 weeks and 6, 12, and 18 months after the first dose. Annual booster doses are recommended for ongoing protection.
- Botulism: Investigational New Drug (IND) shot done annually
- Measles-mumps-rubella (MMR): You need a second dose ONLY if you never got it. This includes also immunization for the Varicella (chickenpox)
- Tdap (Diphtheria/tetanus/pertussis): a single injection of one dose (0.5 mL) by theintramuscular. Booster if necessary if at least 2 years have passed
- Hepatitis A: injection every 6 months
- Hepatitis B: 3 doses — with the second dose given 4 weeks after the first and the third dose 5 months after the second. (Only if you weren't vaccinated as a child or if you haven't taken the adult doses before)
- Human Papillomavirus: three dose series administered at 0, 2, and 6 month intervals. Each dose is 0.5-mL administered as an intramuscularly injection in the deltoid, from 9 through 26 years of age. (unless you were vaccinated before)
- Influenza: there are several types. Annual, plus there are 3 perennial. You need one shot each for the perennials if you didn't have before and one for the annual
- Japanese encephalitis: two injection series administered 28 days apart with annual boosters (pregnant and nursing women can't be vaccinated)
- Lyme Disease: Yearly shot
- Meningococcal: Injection with booster ever 7 years (Robert, I'm sure you had it done, if you can't remember when, we'll do it again)
- Plague: Primary vaccination consists of a series of three injections, boosters every 6 months
- Pneumococcal: a one time 0.5 mL dose administered intramuscularly or subcutaneously. (This you only get again if you reach 65)
- Polio: Adults who have previously completed a primary series of 3 or more doses should be given one dose of IPV. (Again if you already had your adult dose, you don't need too)
- Rabies: Administer three 1-mL doses of vaccine intramuscularly (IM) in the deltoid muscle, one each on days 0, 7 and 21 or 28. Boosters are only given if bitten)
- Shingles: Pill only over 50 year old
- Smallpox: droplet of ACAM2000 by percutaneous route every 10 years
- Tuberculosis (TB): droplet every 10 years
- Typhoid: one capsule every other day, for a total of four capsules. Boosters every 2 years
- Yellow Fever: a single, 0.5-mL dose subcutaneously, boosters every 10 years. Not recommended for people over 65

Each of us has his own schedule. While Sam was handling the process, I don't know if people are keeping track and injecting themselves with the boosters, although there are very few that were needed in the past month.

Marks, the Hammond twins, Merlyn, Queen and you are the only ones who never received the shots, because Sam isn't here, basically, and he would have given you some time to get accustomed before giving you all of those shots. By the way, these were mandatory, so I don't know if you want to take over or let Vance tell people to give their own shots.

The kids have been following the standard recommended schedule. I'm including a copy, which is the same I sent all the other parents.

As you can see, we are a very self-sufficient group that has learned to deal with the lack of doctor (or an incompetent one as far as I'm concerned). However, having spent my adult life in the Air Force, I am well aware of the need of physicals. They are one of the necessary evils in life. I guess we can discuss what tests you want to run when you come to the house and we go from there.

I had not realized that was the impression given. I prefer to be direct about my communications; however, I shall have to reconsider my tone to avoid such misunderstandings in the future. I suppose there are more things to get used to here than simply the training regimen and lack of amenities.

Thank you for the information regarding the vaccines. I had intended to make inquiries as part of the physical exams, but this is most helpful. Agent Hanna was remarkably thorough. I shall have to discuss with the Director what he would prefer regarding future administration of the vaccines. I would not want to assume and he does seem to have very specific ideas regarding how he prefers to do things.

I hope I might someday prove there are some doctors who do genuinely care about helping people. But, until then, we can discuss the tests and go from there, as you say.
