Eventually he settled down in a small American town where he found work as a substitute history teacher at a local high school where he met a young Dean Winchester. Dean was trouble from the start and had been bounced around enough to the point that he didn't care about school or his grades. Little by little, however, the Doctor managed to get past Dean's defenses enough that Dean began to trust him. He even told the Doctor about his life and the Doctor was about to tell Dean about his own... when the Tardis picked up a distress signal. Deciding he could take Dean along, the Doctor tracked him down to the hotel room he'd been living in... only to peek through the window and catch a glimpse of Dean playing with a young Sam. Realizing he was needed more by his little brother than he could ever be needed by the last of the Time Lords, the Doctor left. Dean went into school the next morning (actually looking forward to class now) to find that 'Mr. Smith' was gone and had left no address.
Several years later (which didn't really seem that long between adventures and everything with Martha) the Doctor finds himself in another small American town looking into a series of strange events only to run into Dean and Sam once more. Dean instantly recognizes his teacher and realizes that he hasn't aged a day. Thinking the worst, Dean puts him through a series of tests. After the tests the Doctor admits that he's not a demon or a monster but he's not human either. He finally tells Dean the truth and shows him the Tardis, offering to take both brothers with him.
