I've seen "appearance" done both ways -- as a list of statistics and as a text block. As a player I don't have much of a preference, but I always like when it's included; as a mod, I've chosen to do the appearance section as a free-form paragraph-style section -- I give some suggestions of what to include, but let the player choose what they think is important to write. (Incidentally, the app for the game I'm currently modding is here, if you want to see my application philosophy in action.) I don't think there's a right or wrong way to deal with appearance, really; I just want to get a general sense that the player has thought about what impression her character will make on other characters, which goes a long way toward having a consistent and well-developed character in-game.
Consistency and neatness in the application does matter to me as a player -- if I find when I'm filling it out that the app is drastically unorganized and jumps all over the place, it makes me wonder whether the mod is going to treat the game that way, too? Or something like that. I'm not sure if that's exactly what you're asking, but I do think it's important. :)
