Dr. Anderson,
First, I want to thank you for your letter. I found it very kind of you to write me and ask after me. I find that doctors rarely care about their patients as much as you have already expressed caring, and it's only helped make the following decision easier for me.

I think I would very much like to come in for an appointment. I'm not sure if my mother told you or not, but I've been having issues with my memory. It seems to be very specific (namely, not remembering being involved with someone) and the nature is troubling me. I can get more into depth when I see you.

There's something else I fear someone is stalki I look forward to meeting with you. I'll call your secretary today for an appointment sometime this week.
