It's "Ich gratuliere
dir
zum Geburtstag".
"Gratuliere" needs the dative and therefore you have to ask "Whom do I congratulate?" --> "dir"

"Zum Geburtstag" is right, because the word "Geburtstag" is masculine. "Zur" is for feminine words.
Last but not least: You also could say "Alles Gute zum Geburtstag!" It's slightly less formal than "Ich gratuliere dir zum Geburtstag!"
