As soon as Voldemort fell, Harry shouted, "Now, Hermione!"
In a blink of an eye, Hermione vanished from sight, and Harry prayed to the gods that this would work. He was not going to lose the man who had almost single-handedly helped him win the war.
Hermione arrived just at the point when Harry was leaving the Shrieking Shack, and when he’d gone, Hermione rushed to Severus’ side. The older man had lost copious amounts of blood and his pulse was very weak, but he was still alive.
It didn’t take much effort to Apparate Severus directly into the Hogwarts infirmary. The wards were still down and the whole place was in chaos. Madam Pomfrey remained calm throughout. She treated the wounded, delegated tasks and shouted orders to the newly appointed medical staff.
When Hermione gently lowered Professor Snape onto one of the few vacant cots, Poppy was at his side immediately.
"What’s happened, Miss Granger?" Poppy demanded.
"Professor Snape has been bitten by a poisonous snake. There's not much time, but I have a message for you from Harry. Take care of him. Do whatever is necessary to save him. This man is a hero. Without him, we would have lost the war."
"We haven't won the war yet, Miss Granger."
Hermione smiled at the mediwitch. "Yes, we have." And with that, Hermione disappeared once again, speeding forward through time to the final battle.
